Physiotherapy after a car accident, funded by your auto insurance

If you were hurt in a collision in Ontario, treatment for your injuries, which insurers and clinics call motor vehicle accident or MVA physiotherapy, is paid for through the accident benefits that come with every auto insurance policy in the province. It does not matter who was at fault, whether you were the driver, a passenger, a cyclist or a pedestrian, or whether the other vehicle stopped. What matters is that you start treatment early and that the paperwork is done properly, and both of those are things we do every week at all twelve of our clinics.

This page explains how accident benefits work after the changes that took effect on 1 July 2026, what treatment involves, what it costs you, and what to bring to your first visit. If you would rather just talk to someone, call the clinic nearest you and say it was a car accident. Reception will take it from there.

Who this is for

Most people who come to us after a collision have one of a handful of injuries. Whiplash is by far the most common: the head is thrown forward and back, the neck muscles and ligaments are strained, and over the following day or two the neck stiffens, headaches start and the shoulders ache. Low back pain from the jolt through the seat is next. Then shoulder and chest bruising from the seat belt, knee and hip injuries from bracing against the floor, and the wrist and hand injuries that come from gripping the wheel.

Some injuries take longer to declare themselves. Dizziness, trouble concentrating, light sensitivity and a foggy feeling in the days after a crash can mean a concussion, even without a blow to the head. Numbness or tingling down an arm or leg can mean a nerve is irritated. If you have any of these, tell your family doctor or go to a walk-in clinic as well as booking with us, because the assessment on file will matter for your claim.

You do not need to wait for your insurer to approve anything before booking an assessment. Delay is the main reason simple soft-tissue injuries turn into long-term problems, and the accident benefits system is built to fund early treatment.

How accident benefits work since 1 July 2026

Ontario changed its auto insurance rules on 1 July 2026, and the changes matter for how your treatment is paid for.

Medical and rehabilitation benefits are still mandatory. Every Ontario auto policy still includes medical, rehabilitation and attendant care benefits. Physiotherapy, chiropractic, massage therapy and other treatment for your injuries are paid from these. Most other benefits, including income replacement, caregiver, housekeeping and death benefits, became optional on 1 July 2026. Whether they are in your policy depends on what you bought and on renewal: FSRA’s policy wording says benefits your policy carried before the change continue as optional benefits at renewal unless you declined or changed them in writing. We can only speak to the treatment side; for anything else, check your policy or ask your broker or insurer what it carries.

Your auto insurer now pays first. Before July 2026, extended health plans through work often had to be used up before the auto insurer paid. Since 1 July 2026, the auto insurer is the first payer for medical and rehabilitation treatment, with medication the main exception. In practice this means we bill your auto insurer directly and your workplace plan is left untouched for everything else in your life.

How much is available depends on how your injury is classified. Most sprains, strains, whiplash and similar soft-tissue injuries fall under the Minor Injury Guideline, which funds up to $3,500 of treatment and assessment. If your injuries are more serious, or you have a documented pre-existing condition that will stop you recovering within that guideline, the standard limit is $65,000 for medical, rehabilitation and attendant care combined. Catastrophic injuries have a limit of $1,000,000. Some policies carry higher optional limits. Your physiotherapist’s assessment is the starting point for deciding which applies, and it is the insurer who decides.

Accidents before 1 July 2026 are not affected. If your collision happened before that date, your claim continues under the policy that was in force at the time, including the old payment order.

What you need to do, and by when

The deadlines below are the insurer’s, not ours, and missing them is the most common avoidable problem we see.

• Within seven days of the accident: tell your own auto insurer that you were injured and intend to claim accident benefits. Do this even if the other driver was at fault. Your insurer sends you an application package.
• Within thirty days of receiving the package: return the Application for Accident Benefits, known as the OCF-1. You will also need a Disability Certificate, the OCF-3, completed by a health professional. We complete the OCF-3 at your assessment if we are the first practitioner you see.
• Before treatment beyond the assessment: for a minor injury, we submit a Treatment Confirmation Form (OCF-23) to your insurer through the province’s Health Claims for Auto Insurance system and can start straight away. For anything outside the guideline we submit a Treatment and Assessment Plan (OCF-18), which sets out the treatment, how often and at what cost. The insurer has ten business days to say what it will and will not pay for.
• If you disagree with the insurer: disputes go to the Licence Appeal Tribunal and must be started within two years. We are not lawyers and do not give legal advice, but we will give you complete, timely clinical records to support whatever you decide to do.

Keep a copy of everything. Ask your insurer for your claim number and the name of your adjuster, and bring both to your first visit.

What treatment involves

The first appointment is an assessment, not just treatment. Your physiotherapist takes a history of the collision itself (speed, direction of impact, whether you saw it coming, whether your head hit anything), then examines the injured areas: range of movement, strength, the nerves, and the joints and muscles around the sore spots. This is where a whiplash injury is graded, red flags are checked, and the classification for your claim is set. Bring the police report number, the collision details and any hospital or doctor’s notes.

Early treatment for whiplash and back strain is about keeping you moving safely. Hands-on treatment settles the irritated joints and muscles; gentle exercise starts in the first week, because the evidence is clear that rest beyond the first day or two slows recovery; and you get advice on posture, sleeping positions, driving and work. As pain settles, the programme shifts to strength and endurance so the neck and back can cope with a full day again. Where it helps, your plan can include chiropractic care, massage therapy or acupuncture at the same clinic, and we write it into the treatment plan so the insurer funds it as one programme.

Concussion symptoms are managed alongside a doctor: graded return to screens, work and exercise, treatment for the neck (which causes many of the headaches), and balance work where dizziness lingers. If we think you need imaging, a specialist or a psychologist, we say so and help arrange the referral, because these are also fundable under your benefits.

What accident benefits do and do not cover

Within the limits above, your benefits fund the assessment, physiotherapy, chiropractic, massage therapy and acupuncture set out in an approved plan, plus assistive devices such as a collar or lumbar roll where they are prescribed. They also fund the cost of the forms and reports the insurer requires.

They do not, on their own, make up lost wages: income replacement is now an optional benefit, in your policy only if it was bought or carried forward at renewal, so check your policy. They do not fund treatment the insurer has declined in writing, although you can dispute that. And they do not cover the gap if you choose treatment outside the plan. If your plan is declined or the guideline is used up and you still need care, we will tell you honestly what the options are, including your extended health plan, which is available again once the auto insurer has stopped paying.

Who provides it

Registered physiotherapists lead accident treatment at every one of our clinics. Each is regulated by the College of Physiotherapists of Ontario and is registered as a provider on the Health Claims for Auto Insurance system, which is what lets us submit forms and bill your insurer directly. Where your plan includes them, chiropractors regulated by the College of Chiropractors of Ontario, registered massage therapists regulated by the College of Massage Therapists of Ontario, and registered acupuncturists work alongside your physiotherapist in the same building, so you are not carrying your file between clinics.

Our clinics have been treating motor vehicle accident claims across the Greater Toronto Area, Hamilton and Halton since 1996. Reception staff at each clinic handle accident benefit paperwork daily and know what each insurer expects.

Cost and direct billing

For approved treatment under an accident benefits claim there is nothing to pay at the desk. We bill your auto insurer directly for the assessment, treatment and reports. If your insurer pays only part of an invoice, for example when a treatment plan is partially approved, we tell you before the appointment, not after. There is no HST on physiotherapy, chiropractic or acupuncture; massage therapy carries HST, which is billed to the insurer as part of the approved plan.

If your claim has not yet been set up when you first come in, you can still be assessed. We will explain the private cost of the assessment and, once your claim number arrives, submit it to your insurer for reimbursement.
